What is ECM?

Helping Patients Overcome Social Barriers to Better Health

ECM is a Medi-Cal benefit for Anthem Blue Cross and Health Net members. It connects high-need patients with a Lead Care Manager (LCM) at our clinic who helps coordinate care, support health goals, and remove barriers related to housing, food, mental health, and more.

Who Qualifies?

We serve patients who fall into Populations of Focus (PoF):

Frequent hospital/ER visits due to chronic conditions (asthma, diabetes, HBP) and at risk for the above.
Adults/youth with Serious Mental Illness (SMI) or Substance Use Disorders (SUD)
Individuals at risk of institutionalization or recently discharged from facilities
Pregnant/postpartum individuals in high-risk populations
High utilizers of medical/behavioral services
